PRN Home Infusion Nurse

Infusion Nurse I providing basic direct patient care in the home or alternate infusion site under close supervision. Initiates and implements basic nursing plan of care, administers medications, educates patients and families, and assesses patient needs during home visits. Ensures patient safety and continuity of care under a physician’s plan, documents in real-time, coordinates with the Clinical team, and participates in discharge planning and guidance. Requires travel to patient homes (100% travel) with strong emphasis on HIPAA compliance, documentation accuracy, and collaboration with nurses, supervisors, and support staff. Preferred BSN and 1 year home health or acute care infusion experience.
