Job Summary
Initiate, develop, and implement basic nursing plan of care treatments for patients in the home or alternate infusion sites. Administer medications, educate patients and families, and modify treatment plans based on physician orders and changing patient status. Assess physical needs through health data access and interviews, communicate with clinical teams and providers to ensure continuity, and prepare real-time clinical documentation for billing optimization. Coordinate discharge planning, respond to patient requests, and initiate emergency procedures as necessary. Participate in nursing on-call responsibilities and provide oversight or precepting to paraprofessionals as assigned. Requires active RN license, CPR certification, and willingness to travel 100% of the time.
Required Qualifications
- Active and unrestricted RN license in the state of practice
- Minimum of 0-2 years of infusion nursing experience
- Current CPR certification (in-person class required)
- Demonstrated competency in patient care standards
- Advanced certification and training as applicable
- Required licensure to operate a motor vehicle in the state of practice
- Access to a vehicle for business travel
- Proof of liability insurance
- Basic knowledge of computer operating systems and software applications
- Ability to self-evaluate for annual appraisal
- Ability to jointly set professional growth goals with nurse manager
- Ability to apply knowledge in the effective use of nursing technology tools
- Ability to see up close and from a distance

Desired Qualifications
- Bachelor of Science in Nursing as granted by an accredited school of nursing
- 1 year of previous experience as a RN in a home health or acute care setting
