Job Summary
Initiate, develop, and implement basic nursing plans of care for patients in homes or alternate infusion sites, evaluating progress toward established goals. Administer treatments and medications while educating patients and families to achieve therapy independence. Re-evaluate patient needs through physical reassessment and supplemental physician orders, modifying treatment plans as conditions change. Communicate promptly with the clinical team and physicians regarding significant patient status changes, providing written summaries for order revisions. Prepare clinical documentation in real-time to support billing optimization and coordinate discharge planning with patient instructions. Participate in multidisciplinary team conferences, mentor new nurses, and fulfill on-call responsibilities. Requires active RN license, CPR certification, and 100% travel willingness.
Required Qualifications
- Active and unrestricted RN license in the state of practice
- Minimum of 0-2 years of infusion nursing experience
- Current CPR certification
- Demonstrated competency in patient care standards
- Advanced certification and training as applicable
- Required licensure to operate a motor vehicle in the state of practice
- Access to a vehicle for business travel
- Proof of liability insurance
- Basic knowledge of computer operating systems and software applications
- Ability to self-evaluate for annual appraisal
- Ability to jointly set professional growth goals with nurse manager
- Ability to apply knowledge in the effective use of nursing technology tools
- Ability to see up close and from a distance
- Ability to see color and peripheral vision
- Ability to adjust focus
- Ability to lift up to 50 pounds
- Ability to lift a maximum of 75 pounds
- Ability to exert up to 20 pounds of force occasionally
- Ability to exert up to 10 pounds of force frequently
- Ability to walk, stand, crouch, kneel, turn, pivot, balance, stoop, reach overhead, grasp, push, pull, lift and carry
- Ability to respond to unpredictable home environments
- Willingness to travel 100% of the time for business purposes
Desired Qualifications
- Bachelor of Science in Nursing as granted by an accredited school of nursing
- 1 year of previous experience as a RN in a home health or acute care setting
