Job Summary
Initiate, develop, and implement basic nursing plans of care for patients in homes or alternate infusion sites, evaluating progress toward goals. Administer medications, perform treatments, and educate patients and families on prescribed therapy. Modify treatment plans based on physician orders and reassess patient needs through interviews and physical examinations. Communicate promptly with physicians regarding significant changes and provide written summaries. Prepare clinical documentation in real-time, coordinate discharge planning, and oversee paraprofessionals as needed. Participate in multidisciplinary conferences, complete required training modules, and fulfill on-call responsibilities. Travel 100% of the time to provide direct patient care.
Required Qualifications
- Active and unrestricted RN license in the state of practice
- Minimum of 0-2 years of infusion nursing experience
- Current CPR certification (in-person class required)
- Demonstrated competency in patient care standards
- Advanced certification and training as applicable
- Required licensure to operate a motor vehicle in the state of practice
- Access to a vehicle for business travel
- Proof of liability insurance
- Basic knowledge of computer operating systems and software applications
- Ability to self-evaluate for annual appraisal
- Ability to jointly set professional growth goals with nurse manager
- Ability to apply knowledge in the effective use of nursing technology tools
- Ability to lift up to 50 pounds
- Ability to exert up to 20 pounds of force occasionally
- Ability to exert up to 10 pounds of force frequently

Desired Qualifications
- Bachelor of Science in Nursing as granted by an accredited school of nursing
- 1 year of previous experience as a RN in a home health or acute care setting
