Job Summary
Home Health RN Field Nurse provides in-home nursing care, starting with an initial patient and family assessment, developing and implementing a comprehensive care plan, supervising LPNs and Home Health Aides, administering medications and treatments, coordinating discharge planning, and acting as Case Manager when assigned. Responsibilities include using health data to identify nursing diagnoses, creating education plans, updating plans as patient needs change, and maintaining communication with physicians. Requires active RN licensure and at least 1-2 years of recent acute care experience; a Bachelor's in home health is preferred. Travel is in-home across assigned caseload within the service area.
Required Qualifications
- Graduate of an accredited school of nursing
- Active RN licensure
- 1-2 years of recent acute care experience in an institutional setting
- Must be a licensed driver with a valid automobile and current insurance
