PRN Home Health RN Case Manager

Job Summary
PRN Home Health RN Case Manager responsible for coordinating and managing care for patients in a home health setting, providing skilled nursing services (including wound care and medication administration), developing and updating individualized care plans, supervising home health aides and other healthcare professionals, and communicating with patients, families, and the healthcare team. Requires active RN license, at least 1 year in home health, CPR, strong assessment and communication skills, ability to work autonomously, and a valid driver's license for home visits. Schedule is PRN with Weekdays and On-Call duties; compensation is Paid Per Visit ($80-$100 per visit). Benefits include Cell Phone and Millage Reimbursement.
Required Qualifications
- Current Registered Nurse (RN) license in the state of practice
- Minimum of 1 year of experience as an RN in a home health or related healthcare setting
- Current CPR certification
- Knowledge of OASIS documentation system preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work independently and manage time effectively
- Valid driver's license and reliable transportation for home visits
