Job Summary
The Home Health RN provides patient-centered nursing care in patients' homes, administering medications and treatments, performing wound care and monitoring vital signs, developing and implementing individualized care plans with the healthcare team, educating patients and families on disease management and home care, assisting with mobility and daily living activities, and maintaining accurate records in compliance with regulatory standards. Requires strong clinical skills, communication, ability to manage multiple priorities in fast-paced environments, and capability to work across home health settings with collaboration from LPNs, physicians, and other professionals. Schedule includes PRN weekdays, weekends, on-call, and flexible availability; physical tasks include lifting up to 50 lbs and navigating varying home environments. Valid RN license in the work state required; 1–2 years of RN experience preferred, with home health experience preferred.
