Job Summary

Conduct comprehensive patient assessments, develop individualized care plans, and administer prescribed medications and treatments in patients' homes. Monitor and document patient progress, educate families on disease processes and home care techniques, and provide wound care, catheter care, and health screenings including vital signs. Collaborate with physicians and therapists to coordinate care, ensure regulatory compliance, and maintain accurate patient records while offering emotional support. Requires valid RN license, experience in home health or long-term care, and physical ability to lift up to 50lbs. Flexible PRN schedule covering days, weekends, and optional on-call rotation.

Required Qualifications

  • Valid license as a Registered Nurse (RN) in the state where work is performed.
  • Some nursing experience, preferably in a home health or similar setting.
  • Strong clinical skills and knowledge of nursing principles, procedures, and best practices in long-term care.
  • Excellent communication and interpersonal skills, with the ability interact effectively with residents, families, caregivers, and healthcare professionals.
  • Demonstrated ability to prioritize tasks, manage multiple responsibilities, and adapt to changing priorities in a fast-paced environment.
  • Understanding of regulatory requirements governing nursing practices and home health agencies.
  • Ability to build rapport with patients through a compassionate and caring demeanor.
  • Ability to lift and transfer patients, with or without assistance, up to 50lbs.
  • Capability to perform physical tasks such as bending, standing, walking, and climbing stairs for extended periods.
  • Strength to handle medical equipment and assist patients with mobility and physical activities.
  • Visual acuity and dexterity for performing tasks such as reading medical records, administering injections, and handling delicate medical instruments.
  • Ability to respond swiftly and efficiently to emergency situations.
  • Capacity to work in various home environments which may include navigating stairs, tight spaces, or uneven surfaces.
  • Maintain regular attendance and timeliness as set forth within the work schedule designated by the department supervisor/manager.
  • Job requires accepting criticism and dealing calmly and effectively with high-stress situations.
  • The ability to use your abdominal and lower back muscles to support part of the body repeatedly or continuously over time without 'giving out' or fatiguing.
  • The ability to keep your hand and arm steady while moving your arm or while holding your arm and hand in one position.
