Job Summary
In-home PRN nursing role providing patient-centered care to individuals in their homes. Administer medications and treatments per physician orders, perform wound care and catheter insertion, monitor vital signs, and collaborate with patients, families, and the healthcare team to develop and implement individualized care plans. Document patient progress and changes, educate patients and families on disease management and home care techniques, assist with mobility and activities of daily living, maintain accurate records, and ensure infection control and safety. Coordinate with LPNs, physicians, and other professionals to deliver comprehensive care; respond promptly to changes in condition and provide appropriate interventions. Requires an active RN license in the practicing state; prior home health experience preferred; ability to lift up to 50 lbs and work in varied home environments.
Required Qualifications
- Valid Registered Nurse (RN) license in the state where work is performed
- 1-2 years nursing experience preferred
- Strong clinical skills and ability to perform range of nursing procedures with precision
- Excellent communication and interpersonal skills
- Ability to prioritize tasks and adapt to a fast-paced environment
- Ability to lift and transfer patients up to 50 lbs
- Ability to respond swiftly in emergencies
- Ability to work in various home environments including stairs and tight spaces
