PRN Home Health Registered Nurse (RN)

RN to work PRN, providing one-on-one skilled nursing and education to patients in their home environment. Collaborates with family/caregivers, physicians, and care team; executes each patient’s Plan of Care; completes accurate documentation; participates in case conferences; and serves on on-call rotations as assigned. Ideal candidate has 2+ years of nursing experience (acute or post-acute preferred), active state RN licensure, CPR certification, and a valid driver’s license. Flexible PRN schedule with minimum availability of 6 visits per week and mileage reimbursement; working within LiveWell Partners Network Agency in St. Louis, MO.
