Job Summary
PRN Home Health Physical Therapist in San Antonio, TX providing one-on-one in-home PT. Evaluate patients, develop individualized therapy plans, provide in-home treatments to improve mobility, strength, balance, and functional independence. Educate patients and caregivers on exercises and safety, monitor progress, adjust plans, and complete accurate documentation using EMR (Kinnser/WellSky preferred). Flexible, self-managed schedule with independent work in patients’ homes.
Required Qualifications
- Active Texas Physical Therapist (PT) license
- Home health experience preferred
