Job Summary
Evaluate patients and develop individualized therapy plans. Provide in-home physical therapy treatments to improve mobility, strength, balance, and functional independence. Educate patients and caregivers on exercises and safety techniques. Monitor progress and adjust treatment plans. Complete accurate and timely documentation. Work on the road – Houston and surrounding areas with a PRN, flexible, self-managed schedule. Requires Texas PT license and ability to work independently in patients' homes; EMR experience preferred (Kinnser/WellSky).
Required Qualifications
- Active Texas Physical Therapist (PT) license (required)
- Home health experience preferred
- Kinnser/WellSky EMR experience (preferred)
- Reliable transportation & valid driver’s license
