Job Summary
Provide in-home physical therapy evaluations and treatments to patients; develop and implement individualized therapy plans to improve mobility, strength, flexibility, and functional performance; utilize therapeutic exercises, manual therapy, modalities, and assistive devices; educate patients and families on home exercise programs and recovery strategies; monitor and document progress; collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care; guide use of assistive devices and home modifications to enhance independence; maintain accurate patient records and contribute to care planning and case conferences; PRN scheduling with day shifts and pay-per-visit compensation; license in the work state is required; located in the Bethlehem, PA area.
Required Qualifications
- Valid Physical Therapy license in the state where work is performed
- Some physical therapy experience in a home health or similar setting preferred
- Excellent communication and interpersonal skills
- Ability to manage multiple responsibilities in a fast-paced environment
- Ability to build rapport with patients through a compassionate demeanor
- Ability to lift and transfer patients up to 50lbs
- Capacity to work in various home environments with stairs/uneven surfaces
- Visual acuity and dexterity for medical records and tools
