Job Summary
PRN Home Health Physical Therapist with responsibilities including evaluating, planning, and implementing medically prescribed treatment programs to restore function, relieve pain, and prevent disability in home health patients; responsibilities also include evaluating patient functional level, documenting reactions to treatment, educating patients and families, and maintaining quality therapeutic care; requires WA PT license, CPR, driver’s license, and at least 1 year of PT experience; experience in Medicare Certified Home Health preferred; employed by MultiCare Yakima Memorial Hospital in a PRN capacity.
Required Qualifications
- Successful completion of a physical therapy program
- Licensed in the State of Washington as a physical therapist
- Health Care Provider level CPR certification
- Current Washington state driver’s license
- Minimum of one (1) year of physical therapy experience
- Experience in Medicare Certified Home Health preferred
- Proof of a safe driving record obtained by a motor vehicle report
