Job Summary
Provide in-home physical therapy evaluations and treatment to patients, develop individualized therapy plans, and implement interventions to improve mobility, strength, and daily functioning. Educate patients and families on exercises and home programs; document progress and communicate with supervising physicians. Collaborate with physicians, nurses, therapists, and other healthcare professionals to coordinate care; maintain patient records and ensure regulatory compliance. PRN schedule with weekday and weekend availability; mileage reimbursement included. Must have active PT license in the relevant state and 1+ year experience in physical therapy, preferably in home health; ability to lift up to 50 lbs and perform related physical tasks; strong communication and compassionate patient care.
Required Qualifications
- Valid Physical Therapy license in the state where work is performed
- 1+ year physical therapy experience, preferably home health or similar setting
