Provide skilled in-home nursing visits in Refugio TX, performing vital signs, medication administration, wound care, and catheter care based on the plan of care. Observe and report patient condition changes to the RN Case Manager while educating patients and caregivers on medications, safety, and disease management. Complete accurate and timely documentation and communicate effectively with the care team. Requires an active Texas LVN license, reliable transportation, and valid driver's license. Flexible, self-managed schedule with $45 per visit.
