Job Summary
Direct patient care in patients' homes under RN supervision, assisting with safe transfers/ambulation, reporting changes, and documenting care. Collaborates with physician, clinical team, and patient to support discharge planning; applies infection control principles; maintains a safe, clean environment; promotes respectful communication with patients/families and staff; supports proper nutrition and fluid intake; contributes to agency operations and time management; participates in case conferences and interdisciplinary communication to ensure patient needs and progress; attends 12 hours of in-services per year; demonstrates ongoing professional growth and competency; requires CPR and Hepatitis documentation and completion of the Certified Home Health Aide Program.
Required Qualifications
- Completion of Certified Home Health Aide Program
- 1 year of Home Health Aide experience in a home health agency
- Reliable transportation
- Valid and current auto liability insurance
- Good communication skills to relate to patients, caregivers, and clinical team
- Ability to read, write, and speak English
- CPR and Hepatitis profile
- Aide Competency Evaluation Skills Checklist and Written Skills Test
