Job Summary
Provide direct patient care to patients in their place of residence under the direction of a registered nurse, including assisting with safe transfers and ambulation, reporting changes, and documenting care to ensure continuity of care. Maintain infection control principles and a clean, safe environment; promote respectful communication with patients, families, and the clinical team; support nutritional needs and fluid intake; contribute to efficient operation of the agency; participate in weekly case conferences and interdisciplinary communication to align on patient needs and service delivery; adhere to agency policies and participate in in-services. Requires completion of a certified Home Health Aide program and current CPR/Hepatitis profile, with at least 1 year of Home Health Aide experience and reliable transportation.
Required Qualifications
- Completion of Certified Home Health Aide Program
- 1 year of Home Health Aide experience in a home health agency
- Reliable transportation. Valid and current auto liability insurance
- Good communication skills to be able to relate professionally and positively to patients, caregivers, and clinical team
- Ability to read, write, and speak the English language
- Demonstrates interest in the welfare of ill and elderly
- Proof of current, CPR and Hepatitis profile
- Aide Competency Evaluation Skills Checklist and Written Skills Test
