PRN Field Supervisor

Job Summary
PRN Field Supervisor responsible for planning, coordinating, and documenting non-skilled supervisory visits for clients in Philadelphia and surrounding counties, directly supervising personal care assistants, conducting field evaluations, and providing mentorship and support to field staff. Responsibilities include developing initial care plans upon admission, confirming client care schedules, conducting reassessments, performing direct observation of PCAs for performance evaluations, closing care gaps with home visits, communicating client issues to staff, maintaining positive client relationships, managing daily schedules, and participating in meetings. Requires strong listening, supervisory, interpersonal, and written/verbal communication skills, teamwork, healthcare experience, a valid driver’s license, and the ability to travel in the Philadelphia area.
Required Qualifications
- High School Diploma required
- CNA or HHA certification preferred
- Associate's degree or equivalent industry experience preferred
- At least one year of previous healthcare experience
- Proficiency with Microsoft Office Suite or related software
- Must have access to a reliable automobile, valid driver's license, and insurance
- Lift up to 50 pounds
- Mobility to move within offices and client homes, including stairs
- Effective verbal communication abilities
- Excellent active listening skills and service orientation
- Supervisory skills to coach staff and improve service quality
