PRN Family Nurse Practitioner ( Homevisits/Telehealth)

Conduct comprehensive in-home health risk assessments and telehealth visits to identify active and chronic disease conditions, determine physical, mental, and social needs, and take patient vital signs. Provide written documentation of patient visits per NCQA standards, place case management referrals, and communicate with patients, caregivers, physicians, and vendors to ensure proper diagnosis. Maintain patient confidentiality, observe OSHA Universal Precautions, and attend required meetings and professional development activities. Adapt to differing weather conditions and residential environments while performing duties requiring full body motion and the ability to drive up to 150 miles daily.
