Job Summary
Oversee the complete operation and performance of the delivery of dialysis treatment and care under the contract with the Skilled Nursing Facility, providing clinical oversight and acting as a resource to staff, physicians, and physician assistants. Train and develop Skilled Nursing Facility staff, monitor compliance with ESRD, CMS, and facility policies, and conduct comprehensive patient assessments to build and implement individualized care plans. Coordinate care across settings, ensure continuity of care during transitions, and promote patient education and self-management. Facilitate quality initiatives, participate in IDT and QAPI activities, and contribute to a safety-focused, collaborative facility culture. Maintain open communication with the Administrator, Medical Director, and management, and uphold infection control practices, documentation, and timely, professional care delivery.
Required Qualifications
- Graduate of an accredited nursing program
- Associates Degree in Nursing (ADN) or higher (BSN/MSN)
- Current state RN licensure
- Current CPR certification (AHA BLS)
- 18 months of clinical nursing experience with dialysis patients within the last 24 months
- Ability to communicate effectively in English (reading/writing/speaking)
- On-call status ability
- Computer proficiency
