Job Summary
Licensed Master Social Worker (LMSW) to conduct psychosocial evaluations and collaborate with an interdisciplinary dialysis team. Responsibilities include psychosocial assessments, care planning to meet rehabilitation and treatment goals, delivering psycho-educational counseling to improve adherence and quality of life, assisting with transplant-related planning, facilitating advance directives discussions, guiding referrals to community resources, and maintaining KDQOL-informed care. Must stay current with ESRD guidelines and healthcare standards, communicate effectively with patients and the care team, and support adherence and education tailored to individual learning needs.
Required Qualifications
- Master’s degree from an accredited graduate school of social work
- Minimum of 12 months professional MSW experience in a dialysis setting
- State licensure, if required or available in the state where facility is located
- Excellent verbal and written communication skills
- Computer proficiency
