Job Summary
Provide personal care and restorative services for patients in end-of-life care, including activities of daily living, skin care, mobility, nutritional support, and vital signs monitoring. Perform respiratory care, read CADD pump settings, report pain ratings, and maintain patient confidentiality under the supervision of a Unity RN. Document care in the Activities of Daily Living Plan of Care and immediately report changes in patient condition or family situations to the interdisciplinary team. Maintain required competencies, comply with all state and federal regulations, and adjust to frequently changing demands. Share in providing 24-hour, seven-day-a-week coverage with a minimum commitment of 16 hours per month.
Required Qualifications
- Completion of a certified nursing assistant program
- Must be listed on the Wisconsin Nursing Assistant Registry and be eligible to work at state and federally certified facilities
- Valid driver's license, auto insurance, and access to a reliable vehicle
- Computer proficiency (MS Office – Word, Excel, and Outlook)
- Highly motivated and organized with a strong attention to detail and analytical skills to help solve problems
- Ability to communicate effectively, both verbally and in writing
- Ability to maintain confidentiality and discretion in business relationships and exercise sound business judgment
- Ability to keep up to date with policies, practices, and regulations
- Ability to prioritize and handle multiple tasks and capable of dealing with deadline pressures
- Ability to adjust to frequently changing demands
- Completion of mandatory training requirements regarding local, state, and federal regulations and/or Unity's policies and procedures
Desired Qualifications
- Flexibility to pick up shifts that fall on weekends or holidays
- Willingness to work day, PM, and night shifts as needed
- Cross-training to the community
