PRN/As Needed Care Manager

Assess, plan, advocate, and coordinate care from admission to discharge while ensuring a safe transition post hospitalization. Perform duties consistent with the Mercy mission, values, and service standards. Utilize performance metrics and best practices to oversee length of stay and maintain patient experience, safety, and quality of care within the interdisciplinary Care Management model. Requires a BSN, current nursing license, and BLS certification.
