Provide one-on-one skilled nursing care to medically fragile patients in their homes, ensuring they can live with their families instead of in a hospital. Maintain accurate, up-to-date charting and communicate respectfully with patients and their families while collaborating with other healthcare providers. Offer compassionate presence and positive attitude to support the whole household. Shifts range from 8 to 12 hours, available as part-time or full-time positions.
