Private Duty Registered Nurse

Job Summary
Administer nursing care for clients in their place of residence, completing initial and ongoing assessments to identify physical, psychosocial, and environmental needs. Develop, initiate, and revise the Plan of Care as necessary to ensure quality and continuity, while regularly re-evaluating client outcomes and preventing illness or relapse. Provide effective communication to clients, families, and the interdisciplinary team, including regular participation in Case Conferences. Coordinate services with physicians and monitor assigned cases to ensure compliance with third-party payer requirements. Supervise, teach, and provide clinical direction to other nursing personnel such as LVNs, LPNs, HHAs, and CNAs. Perform on-call responsibilities and provide on-call services to clients and families as assigned.
Required Qualifications
- One (1) year experience as an RN in a clinical care setting
- Completion of an accredited Registered Nursing Program
- Current driver's license in good standing
- Current State professional license as an RN in any/all applicable states
- Current CPR
- Must read, write and comprehend English
- Prolonged standing/walking
- The ability to lift/transfer up to 50 lbs
- Ability to meet deadlines and patient needs, make quick decisions, and allocate resources under stressful conditions
- Meet patient/family individualized psychosocial needs
- Requires hand-eye coordination and manual dexterity
- Reliable transportation and auto liability insurance
- Ability to travel locally and work a flexible schedule
- Ability to make visits
- If required to make visits, Hepatitis profile
Desired Qualifications
- Preferred Home health experience
- Preferred, BSN
