Job Summary
Deliver skilled nursing care to assigned patients in private homes, adhering to physician orders and nursing care plans. Perform specific treatments and medication administration while maintaining aseptic technique. Identify new problems or changes in patient status, notify the Director or physician as required, and document all interventions and clinical observations accurately. Function as a team member under the direction of the registered nurse, ensuring all physician collaboration is recorded in the medical record. Adhere to agency policies, complete mandatory training, and maintain a professional appearance and punctuality.
Required Qualifications
- Graduation from an approved certification/program/accredited school/undergraduate/graduate school or post graduate education
- Valid/unencumbered verification/license to practice the job as described in the state where services are provided
- Professional experience as required per state regulations
- Successful completion of assigned screening/competency testing with a proficiency score of 80% or greater
- Reliable transportation
- Current TB results or Chest x-ray
- Current CPR certification
- Ability to exercise independent and sound judgment
- Ability to regularly, sit, stand, stop, bend, kneel, walk, balance, crouch, crawl, push, pull and reach with hands and arms
- Vision and Hearing capabilities within normal limits, with or without aid
- Ability to frequently lift and or move up to 15 pounds
- Ability to occasionally lift or move 100 pounds or more if applicable
- Ability to perform the anticipated level of physical assist required to provide safe client care
- Ability to travel frequently to complete home health visits
