Private Duty Nurse (RN/LPN): All Shifts!

Provide one-on-one skilled nursing care for medically complex pediatric and adult patients in private homes, adult family homes, and assisted living communities. Perform ongoing nursing assessments, implement individualized Plans of Care, administer medications and treatments, and monitor patient status to report changes. Collaborate with physicians and interdisciplinary teams while educating patients and families on disease management and treatment plans. Maintain accurate electronic medical record documentation and ensure compliance with agency policies and state regulations. Participate in quality improvement initiatives and perform other duties as assigned, including advanced interventions such as ventilator management, tracheostomy care, and wound care. Full-time or part-time shifts available with flexible scheduling options.
