Job Summary
Assess, plan, implement, and evaluate patient care for assigned individuals in private homes according to physician orders and nursing care plans. Administer medications, perform specific treatments, and observe aseptic technique while documenting all interventions, new problems, and status changes in clinical notes and supplemental orders. Communicate significant patient status changes to the Director of Clinical Services or Clinical Manager and request necessary order modifications based on skilled nursing judgment. Adhere to agency policies, complete mandatory in-services, and maintain a professional appearance and punctual attendance while traveling frequently to complete home health visits.
Required Qualifications
- Graduation from an approved certification/program/accredited school/undergraduate/graduate school or post graduate education
- Valid/unencumbered verification/license to practice the job as described in the state where services are provided
- Professional experience as required per state regulations
- Successful completion of assigned screening/competency testing with a proficiency score of 80% or greater
- Reliable transportation
- Copy of current TB results or Chest x-ray
- Copy of current CPR certification
- Ability to be flexible, organized and have the ability to exercise independent and sound judgment
- Ability to regularly, sit, stand, stop, bend, kneel, walk, balance, crouch, crawl, push, pull and reach with hands and arms
- Vision and Hearing capabilities within normal limits, with or without aid
- Ability to frequently lift and or move up to 15 pounds and occasionally lift or move 100 pounds or more if applicable
- Ability to perform the anticipated level of physical assist required to provide safe client care
- Ability to travel frequently to complete home health visits
