Job Summary
Provide expert, compassionate 1:1 skilled nursing care including ADLs, vital monitoring, wound care, and medication management for private clients. Coordinate medical supplies, DME, and transport to appointments while maintaining clear documentation and communicating with families and physicians. Deliver personalized health education and training to empower patients. Requires an active LVN license, 2+ years of experience, CPR/BLS certification, and U.S. work eligibility. Shifts range from 2-hour visits to 12-hour blocks with flexible scheduling.
