Job Summary
Provide licensed practical nursing care in home settings for children and adults, ensuring outstanding clinical outcomes through compassionate delivery. Maintain active, unencumbered state licensure and current CPR certification with hands-on component. Conduct electronic documentation via tablet devices provided in each patient's home. Adhere to all health-related requirements for Medicare and Medicaid compliance, including vaccinations and testing. Requires reliable transportation and current TB skin test within the last 12 months. Flexible scheduling available for full-time, part-time, or PRN shifts including days, nights, and weekends.
Required Qualifications
- Active, unencumbered license (LPN/LVN) in the state in which the clinician will practice
- Current CPR certification (with hands-on component)
- TB skin test (current within last 12 months)
- Reliable transportation
- Compliance with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing, subject to exemptions for medical or religious reasons
Desired Qualifications
- Six months prior hands-on nursing experience
