Job Summary
Deliver compassionate homecare to children and adults, ensuring outstanding clinical outcomes through direct patient support. Maintain accurate electronic documentation using the provided tablet in each patient's home while adhering to all health-related requirements for Medicare and Medicaid compliance. Support the team in fostering a workplace culture that celebrates diversity and encourages connection. This role offers flexible scheduling including full-time, part-time, and PRN shifts across days, nights, and weekends. Required Qualifications
- Active, unencumbered license (LPN/LVN) in the state in which the clinician will practice
- Compact licenses must be transferred to your state of residence within 90 days
- Current CPR certification (with hands-on component)
- TB skin test (current within last 12 months)
- Reliable transportation
- Compliance with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing, subject to exemptions for medical or religious reasons
Desired Qualifications
- Six months prior hands-on nursing experience
