Job Summary
Provide licensed practical nursing care in home settings for children and adults, ensuring outstanding clinical outcomes through compassionate delivery. Maintain active, unencumbered LPN/LVN licensure in the state of practice and complete current CPR certification with a hands-on component. Perform electronic documentation via tablet devices while adhering to all health-related requirements for Medicare and Medicaid funding, including required vaccinations and testing. Reliable transportation is required for daily patient visits.
Required Qualifications
- Must have and maintain an active, unencumbered license (LPN/LVN) in the state in which the clinician will practice
- Compact licenses must be transferred to your state of residence within 90 days
- Current CPR certification (with hands-on component)
- TB skin test (current within last 12 months)
- Must have reliable transportation
Desired Qualifications
- Six months prior hands-on nursing experience preferred but not required
