Job Summary
Provide 1:1 in-home skilled nursing care per the provider's plan of care, administering medications, performing wound care, and monitoring patient vitals and respiratory status. Assess, document, and report changes in condition while educating clients and families on routines and symptom monitoring. Partner with the care team to support goals of care, coordinate supplies, and ensure timely shift handoffs. Work consistent 1:1 private duty shifts supporting extended coverage, with no overtime required.
Required Qualifications
- Graduate of an accredited practical (vocational) nursing program
- Current LPN/LVN license in the state(s) in which you practice
- One year of clinical experience
- home care/private duty experience
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites and/or work locations
- Valid driver's license
- Proof of auto liability insurance
- Able to visually and aurally observe and assess the patient
- Able to frequently lift, reposition, and transfer patients
- Medium physical activity may require occasional lifting, pushing, or pulling up to 50 lbs
Desired Qualifications
- One year of clinical experience preferred
- home care/private duty experience a plus
- Trach and/or ventilator experience
