Job Summary
Provide nursing and personal care in alignment with the Plan of Care, including medication administration, transfers, positioning, and catheter care. Collaborate with clinical leadership to ensure safe service delivery, perform occasional client assessments, and promptly report medical status changes or emergencies. Complete required clinical documentation accurately using the electronic medical record system and confirm weekly schedules with Care Coordinators to maintain staffing levels. This 24/7 live-in role requires proficiency in tracheostomy and tube feeding care, daily travel to patient locations, and adherence to universal precautions. Required Qualifications
- Current, active RN license in the state of practice, in good standing
- Proficiency with OASIS documentation
- Must be willing stay at the patient's residence on a 24/7 basis
- Proficiency in tracheostomy and tube feeding care
Desired Qualifications
- Minimum of one year of clinical nursing experience preferred
- home health experience desirable
