Private Duty Home Health LPN / RN

Provide in-home direct patient care according to the provider's prescribed plan of care while closely monitoring progress and recovery. Assess and document patient conditions including vitals, blood work, and glucose levels; administer medications, insulin, and IV fluids with thorough documentation. Inspect wounds, change dressings, and assist with personal hygiene while assessing for injuries, infections, or bedsores. Collaborate with providers, aides, and allied health professionals to ensure exceptional patient outcomes. Requires current unrestricted RN or LPN license, CPR/AED/BLS/First Aid certification, and reliable transportation. Practical trach or ventilator experience preferred. Full-time, part-time, or PRN schedules available with flexible hours and daily pay options.
