Private Duty CNA/HHA

Assist clients in their place of residence with personal healthcare and health maintenance activities, including transferring, feeding, mobility, and exercises as outlined in the Home Health Aide Care Plan. Communicate effectively with administrative staff regarding client needs assessments and report significant changes in client condition. Contribute to continuity of care through effective communication with all care team members and regular participation in Case conferences and Team Meetings. Apply infection control and universal precautions during all client care activities to protect staff and patients. Maintain competency by participating in in-service programs and meeting mandatory continuing education requirements. Requires one year of experience as a home health aide or certified nursing assistant in a home health setting, a valid HHA certificate, current CPR certification, and a reliable driver's license with auto liability insurance. Works in patients' homes under various conditions with possible exposure to blood-borne pathogens and infectious diseases.
