Job Summary
Our Home Health Aides provide in-home patient care as a Private Duty Caregiver, supporting the Plan of Care by monitoring patient condition, providing personal services, assisting with ADLs, grooming, meal preparation, and light housekeeping. You’ll monitor and record patient status, collaborate with the care team, educate families, and maintain EMR documentation. Responsibilities include on-call coverage, ensuring a safe home environment, following professional standards and agency policies, and continuing education as required. Must hold a Home Health Aide Certificate or equivalent, have 1+ year of relevant experience, CPR, and a valid driver’s license; strong English communication skills are required.
Required Qualifications
- High School graduate or equivalent
- Home Health Aide Certificate or equivalent certification in good standing
- Minimum of one year (1+) of experience as a home health aide or CNA
- Active CPR certification
- Valid driver’s license, reliable transportation and insurance
