Principal Care Management (PCM) Nurse

Job Summary
Maintain ongoing communication with patients enrolled in the Principal Care Management program, monitoring symptoms, medication adherence, and treatment progress. Provide patient education on their conditions and care plans, assist in implementing follow-up care plans, and coordinate services to ensure continuity. Identify changes in patient status and escalate concerns to the provider while accurately documenting all interactions in the system. Ensure patients follow treatment recommendations and scheduled appointments. Requires a licensed nurse or MD with advanced English skills and experience in chronic care management. Full-time, remote role with U.S. business hours (EST), Monday through Friday, 9:00 a.m. to 7:00 p.m. Florida time, 45 hours per week, including lunch and shift breaks.
Required Qualifications
- Licensed Nurse or MD
- Advanced English level (C1 or higher) – excellent spoken and written communication
- Proven experience working with U.S.-based medical offices
- Experience with chronic care management, patient follow-up, or care coordination
- Strong clinical judgment and ability to assess patient conditions remotely
- Experience providing patient education and support
- Highly organized with strong documentation skills
- Comfortable working independently in a remote environment
- Bilingual in Spanish and English
- Reliable computer (8GB RAM minimum)
- Stable internet connection (minimum 100 Mbps)
- Quiet and professional workspace
- Headset with microphone
- Monday through Friday, 9:00 a.m. till 7:00 p.m. Florida time (ES)
- 45 hours per week
- 1-hour lunch break and one 30-minute break per shift
