Primary Support Professional

Collaborate in clients' homes and communities to deliver personalized care, ensuring Individual Support Plans are implemented and daily activities are coordinated. Participate in support plan meetings, assist with service monitoring visits, and train staff on client medical needs and medication compliance. Oversee daily documentation, health tracking, and the maintenance of a safe, clean living environment while managing household supplies. Work closely with the Program Coordinator to ensure compliance with medical and training requirements. This role requires a high school diploma, one year of experience in intellectual/developmental disabilities, a current driver's license, and successful completion of background checks.
