Job Summary
Primary Service Coordinator for the MYPAC program, responsible for a caseload of 10 to 12 youth/families, coordinating wraparound care, facilitating the Individual Service Plan (ISP) and the Individualized Crisis Management Plan (ICMP), and ensuring accurate EMR documentation. Roles include providing transportation to appointments, on-call crisis support, and attendance at three weekly meetings, with a flexible, non-traditional schedule based on youth and family availability. Requires a current, valid driver’s license, automobile for work, and proof of auto insurance; mileage is reimbursed as commutes can range up to 60–80 miles. The position supports in-home individual and family therapy and medication management within the Mississippi Youth Programs Around the Clock (MYPAC) framework and emphasizes family-driven, youth-guided services within a home- and community-based Medicaid program.
Required Qualifications
- Master’s degree in a social services discipline (preferred)
- Bachelor’s degree in a social services discipline (required)
- Must have or be eligible for Mississippi's DMH credentialing (required)
- Experience working with at-risk youth and/or families (preferred)
- Clinical experience (preferred)
- Strong organizational skills and attention to detail
- Excellent written, verbal, and oral skills
- Ability to manage multiple priorities simultaneously
- Basic computer knowledge
- Schedule flexibility based on youth/family availability
