Job Summary
Primary Care Nurse role providing patient-centered care under the direction of the primary care operations team. Partners with patients, providers, and care team to drive quality outcomes for an attributed panel. Duties emphasize safe, timely, efficient care with emphasis on triage, computer-enabled workflow, and adaptability to a hybrid after initial on-site training. Requires licensure as LPN or RN, with preferred RN/BSN pathway and Epic EMR proficiency. Schedule includes on-site training, then hybrid model (on-site five days per pay period) with no nights/weekends/holidays after training; monthly on-site clinic meetings; 0800-1630 hours; Mercy health system context and benefits highlighted.
Required Qualifications
- LPN Diploma (Diploma in Practical Nursing)
- Active LPN license
- Active RN license
- Experience: 2-3 years preferred (RN or LPN)
- Education: RN BSN preferred or equivalent (BSN or higher)
- Epic EMR proficiency
- On-site training and subsequent 50/50 hybrid work schedule
