Primary Care RN or LPN

Participate in primary care operations by partnering with providers to deliver safe, timely, patient-centered care for an attributed panel. Manage triage calls and messages for the clinic while maintaining computer competency for extensive electronic work. Complete a minimum of three months of on-site training before transitioning to a 50/50 hybrid schedule, with full-time co-workers working on-site five days per pay period. Attend monthly on-site clinic meetings and maintain licensure as an RN or LPN with 2-3 years of experience. Proficiency with Epic EMR is highly preferred.
