Primary Care RN or LPN

Manage triage calls and messages for the clinic and partner with primary care providers to drive quality outcomes for an attributed panel. Work directly with Mercy Primary Care Providers to deliver safe, timely, patient-centered care consistent with Mercy Service Standards. This newly created role requires completing three months of on-site training before transitioning to a 50/50 hybrid model, with full-time co-workers working on-site five days per pay period. Monthly on-site clinic meetings are also required. Proficiency with Epic EMR and timely patient triage skills are essential for success in this position.
