Primary Care RN or LPN

Manage triage calls and messages for Mercy Primary Care Providers while partnering with the clinical care team to drive quality outcomes for an attributed panel. Work directly with providers to deliver safe, timely, effective, and patient-centered care in a systematic manner. This newly created role requires completing three months of on-site training before transitioning to a 50/50 hybrid schedule, with full-time co-workers expected on-site five days per pay period. Monthly on-site clinic meetings are also required. Proficiency with Epic EMR and timely patient triage skills are essential for success in this position.
