Primary Care RN or LPN

Primary Care Nurse (RN or LPN) collaborates with Mercy Primary Care Providers to deliver patient-centered care in a clinic setting. Under the direction of the primary care operations team, the nurse will partner with patients, providers, and other clinical care team members to drive quality outcomes for an attributed panel. Responsibilities include on-site nursing duties, proficiency with Epic EMR, timely patient triage, and adaptation to a 50/50 hybrid work model after a required on-site training period; role requires on-site attendance for monthly clinic meetings and regular full-time participation, with schedule Monday–Friday and no weekends/holidays after transitioning to hybrid.
