Job Summary
Partner with primary care providers to deliver safe, timely, patient-centered care for an attributed panel. Manage triage calls and messages for the clinic while working directly with Mercy Primary Care Providers. Perform duties consistent with Mercy Service Standards, utilizing proficiency with Epic EMR and timely patient triage skills. This newly created role requires completing three months of on-site training before transitioning to a 50/50 hybrid model, with full-time co-workers working on-site five days per pay period. Required Qualifications
- Diploma in Practical Nursing
- Active LPN license
- Associate of Applied Science in Registered Nursing
- Graduation from registered nursing program
- Bachelor's degree in nursing (BSN)
- 4 years of commensurate experience in lieu of the bachelor's degree
- Active RN license
- Monday – Friday, 0800-1630 schedule
- No nights, weekends, or holidays
- Minimum of three months of on-site training prior to transitioning to a hybrid work-from-home schedule
- 50/50 hybrid model after training
- Full-time (1.0 FTE) co-workers expected to work on-site five days per pay period
- Available to attend monthly on-site clinic meetings
Desired Qualifications
- 2-3 years of experience preferred
- Proficiency with Epic EMR system
- Timely and thorough patient triage skills
- Computer competency
- Excellent time management and prioritization
- Adaptability in a new and changing environment
