Primary Care Registered Nurse

Assess, treat, and educate patients on various medical conditions while administering vaccinations, medications, and treatments. Delegate patient care activities to ensure smooth flow, triage acute symptoms via telephone or in person, and perform point-of-care testing. Coordinate care outside the organization by managing medication reviews, diagnostic results, and follow-ups on ER visits or consults. Document all patient care in the EHR and modify care plans as age appropriate. Provide nursing-centered services through chronic disease and family planning visits, utilizing health literacy approaches to support patient self-management.
