Job Summary
Provide high-quality primary care to the community, empowering patients to take an active role in their health and promoting wellness behaviors. Manage chronic conditions through follow-up care while maintaining effective communication and referring patients to specialty services as necessary. Offer clinical supervision to mid-level providers and nursing staff, and participate in quality assurance initiatives alongside state and national disease prevention efforts. This role is part of a leading multi-site Federally Qualified Health Center serving central Vermont, featuring Level III Medical Homes with advanced panel management and an integrated behavioral health program.
Required Qualifications
- Completion of medical school, residency, and possibly fellowship
- Active and unrestricted medical license in Vermont
- Board certification in Family Medicine or a related specialty
- Strong clinical skills with a patient-centered approach to care
- Excellent communication and organizational skills
- Ability to work collaboratively in a multidisciplinary team
