Primary Care Provider (On-Call, As Needed) - Bilingual

Provide comprehensive primary care including preventive care, chronic disease management, and acute care. Conduct medical assessments, develop treatment plans, and coordinate referrals. Collaborate with a multidisciplinary team, including nurses, behavioral health providers, and care managers. Maintain accurate documentation in the Electronic Health Record (Epic preferred) to ensure continuity of care. Participate in quality improvement initiatives to enhance patient outcomes. Advocate for health equity and culturally responsive care. (For Physicians) Supervise and mentor medical students or residents.
